Clinical Evaluation of Complication of Peptic Ulcer



Abstract
Many literatures have born reported about peptic ulcer and it¢¥s complication, of which gastroduodenal perforation, gastroduodenal hemorrhage and pyloric stenosis are major complications.
This study has designed to get the clinical findings and beat method of treatment for these complication.
The present study summarizes the results of the first 8 years of investigation about major complications of peptic ulcer, as follows.
1. 60 percents of hemorrhage was distributed between age of 41 and 60 years but peak incidence of perforation and obstruction were from 31 to 50 years, as 62.5 percents in former and 67 percent in latter.
2. In the sex distribution, the perforation was predominent in male than female in the ratio 28 : 1, but same comparison was 5.5 : 1 in the hemorrhage and 3.5 : 1 in the obstruction.
3. Incidence of the perforation and hemorrhage were more frequent in the Spring and Autumn.
4. Site of the perforation and hemorrhage was more frequent in the stomach than duodenum.
5. 28 of 39 cases combined with hematemesis and melena. In the 93 cases of the pyloric stenosis, 25 cases had hypochloremic alkalosis and 44 cases hypopotassemia.
6. Vagotomy and pyloroplasty had best results in the emergency treatment of the duodenal perforation and hemorrhage.
7. The over-all operative mortality of the perforation was 12 percents and hemorrhage 5 percents but 2 percent in pyloric stenosis.
